To make your initial visit here easier, we are providing you a registration form and a personal history form, and a records release authorization to fill out to bring with you to your appointment. The history form includes a place to list all your medications, or you may bring your medications with you.

Please bring your insurance cards and copay, if required by your plan, to your visit. Please bring your prescription card as well as a photo I.D. (driver’s license is acceptable). If your insurance company requires a referral from your primary doctor, please give them a call so that your visit will be covered. If you do not receive a packet from us, please call our office at 860-358-2220 and we will send one to you. If you prefer, you may download these forms to print and fill out.
